Panda Health is excited to announce the addition of our newest category: Chronic Care Management+

Products in this category address the requirements of participating in the three care management services that CMS (Centers for Medicare & Medicaid Services) currently offers: 

  • Principal Care Management (one chronic condition)
  • Basic Chronic Care Management (two or more chronic conditions)
  • Complex Chronic Care Management (two or more chronic conditions)

Additionally, Panda Health not only considered the requirements for participation in these three CMS programs but also for products that would allow for health systems to provide better care for patients diagnosed with one or more chronic illnesses – especially patients enrolled in a value-based care contract (i.e., Medicare Advantage plans that have process-based quality metrics). 

What is Chronic Care Management+?

Chronic care management+ (CCM+) is a type of care coordination that provides patients with two or more chronic conditions with ongoing support and monitoring outside regular office visits. CCM solutions can help patients achieve a reasonable quality of life by providing resources and support to manage their conditions. 

Additional CCM+ services can include:

  • Developing and maintaining an individualized comprehensive care plan (ICP) that lists the patient’s health problems and goals, other providers, medications, community services, and other information about the patient’s health.
  • Tracking and documenting patients’ vital signs, symptoms, and medication adherence in real-time.
  • Facilitating communication and collaboration between care team members.
  • Tracking prescription refills and ensuring patients are taking medications as prescribed.
  • Facilitating care pathways and tasks specific to certain disease states.
  • Providing patients with information and resources to manage their health better sets and achieve health goals gives them a sense of purpose and motivation.
  • Billing for specific CCM CPT codes for reimbursement.

These solutions can also include help with self-management education and access to community resources.
